Additional Documentation Required to Apply:

Proof of Funds

Regardless of the amount you seek, you’ll need to have an equal share of funding available.

Letters of Recommendation

The application requires a letter of recommendation from:
(1) The executive director or leader of your organization.
(2) A community leader who can attest to your organization’s ability to execute your project.

WHAT KIND OF PROJECTS ARE WE LOOKING FOR?

There are three categories of projects we’d like to fund:

Play Healthy

a park, a playground, sports equipment, health and fitness classes.

Eat Healthy

after-school meal initiative, meals-on-wheels effort, food donations or kitchens for homeless, cooking classes, organic community gardens.

Stay Healthy

employee wellness programs, healthy living classes, motivation applications (e.g. Fitbits), tobacco-cessation efforts, caregiver training.

WHAT IF I HAVE AN IDEA FOR MY COMMUNITY BUT NOT A PROJECT?
